You can use PATCHS to make admin requests, such as requesting sick notes, referrals or other queries. In order to do this, log in to PATCHS, select whether the request is for yourself or another patient, and then select ‘Admin Request’.
Please do not use PATCHS to request repeat prescriptions and blood results. You can find more information on how to access your test results here.
If you can’t contact us online (via a computer or smartphone), please call the surgery between 8:00am and 6:30pm and select Option 1 to discuss your admin request.
