Register with the Practice

Thank you for your interest in registering as a patient within the Middlewood Partnership.

We are a relatively young organisation (formed in 2019) but we came out of four very well established practices which have been part of our communities for many years.

We aim to offer all of our patients, old and new, a variety of services to support your health and well being.

Temporary Residents

Temporary residents can be seen at the surgery with and are required to make appointments as regular patients do. Please note that temporary residents may be asked to attend the surgery in advance of their appointment to fill out registration paperwork.

Once you are registered as a temporary resident you can receive care for up to 3 months.

Process to Join Middlewood

Step 1 – Catchment Area

Please check that you live within our catchment area by putting your postcode into the map.

We have a wide catchment area as outlined in the diagrams and maps. This means that if you live within our catchment area, for general medical services (i.e. normal services delivered by GP practices), we will be able to provide our full range of services, even if you do not live in the Cheshire East local authority area. We would wish to inform prospective and current patients registered within our practice boundary/catchment area, but outside of Cheshire (such as Stockport or Derbyshire), that access to some other NHS and non-NHS services may be restricted. This is particularly the case for people who may have complex social issues, mental health issues, development issues and/or require additional public services funded or organised at local authority levels. Whilst we, as your GP practice, will do everything within our means to meet your general medical needs, other service provision may be restricted. If there is risk of requiring additional services, registering with a practice within your local authority area may be more beneficial.

If you don’t live within this area, we cannot accept you as a patient at the current time.

If you wish to register for patient access to order repeat medications or view your medical record online, complete the Patient Access Questionnaire – once completed return to one of our bases either in person or via email cmicb-cheshire.middlewood.patientaccess@nhs.net along with copies of two forms of identification – one photographic (such as passport or driving licence) please take this photo whilst holding the document next to your face. The other is to be a proof of address (such as bank statement, council tax or utility bill). Please note these documents will be double deleted once seen and authorised by a member of staff.

Choose Your Base

We are four practices within our organisation based within Bollington, Poynton and Disley. As part of the registration process you will be asked to choose a preferred base practice. This might well usually be based upon your home town but not necessarily.

Although this may be your usual practice, you can receive the benefits of being registered as a Middlewood patient by using any of the services which we offer such as specialist clinics which may not be hosted at your usual practice base.

Step 3 – Confirmation

Once our patient administration team have received your application, you will receive notification from them to confirm that you will now be registered as a Middlewood patient. If you require any assistance in this process then please phone any of the practice bases and ask to be transferred to the patient records team who will be delighted in helping you.  All of the forms are available to be picked up in paper form in person but at present, we are encouraging people not to attend the practice sites if at all possible.
