Self Referral For Adult Mental Health Support

What is the Mental Health service?

The service offers an inclusive, timely approach to mental health support that encourages patients to share decisions about their care and support.

Interventions from the mental health practitioners include psychosocial assessment, brief interventions, brief course in CBT (Cognitive Behaviour Therapy), behavioural activation and CFT.   We can also help with anxiety management, psychoeducation, signposting to other services/organisations, treatment advice/support, and onward referrals where indicated.

We can advise on a wide range of mental health conditions, including psychosis, anxiety, depression, Obsessive Compulsive Disorder (OCD), Post Traumatic Stress Disorder (PTSD), self-harm, drugs and alcohol, personality disorder traits and complex trauma.

 

How do I access the service?

If you are a Middlewood patient that is over 18 years of age, you can self-refer by filling out the form below. External agencies and healthcare professionals can also refer a patient via the form.

The Mental Health service is not a rapid response service. If you or someone you know is experiencing mental health difficulties, or is in crisis outside of the working hours of 7:30 am to 17:00 pm, please call the crisis number on 0800 145 6485 to access support.

If you are not aged over 18, your referral will be rejected.

We do our best to make initial contact within 3 working days of receiving your referral, this may be longer during holiday periods.
