What is the Heart Failure Review Service?

The Heart Failure Review Service provides an annual review for patients with an established diagnosis of heart failure. The review takes place with one of our specialist nurses after blood tests and blood pressure measurements have been completed. We assess your symptoms, risk factors, and medications, ensuring your care is in line with the most recent evidence-based guidelines. If necessary, we can make immediate changes or consult with our Cardiology colleagues at Macclesfield or Stockport.

Who is this service for?

This service is for all patients diagnosed with heart failure, whether or not they are currently under cardiology follow-up. If your GP suspects heart failure, they may refer you to this service for initial assessment and to coordinate further investigations with our Cardiology colleagues, if required.

 

How do I access this service?

If you have an established diagnosis of heart failure, you will be invited for an annual review. You will receive an invitation to schedule a 30-minute appointment with one of our specialist nurses.

 

Where is the service provided?

We currently offer this service at Bollington Medical Centre and Mcilvride Medical Practice. We hope to extend the service to The Schoolhouse Surgery in Disley in the near future.

 

How long is the appointment?

The annual review appointment lasts for 30 minutes.

What can I expect to happen?

At your appointment, we will review your recent blood tests, blood pressure, and weight. We’ll ask about your condition and any difficulties you’re experiencing with daily activities. The medications you are taking will be assessed for appropriateness, and adjustments will be made if needed based on the latest medical evidence. This is also an opportunity for you and your family to ask questions and receive education on important aspects such as symptom red flags, medication management, and lifestyle tips.

As an outcome of the review, with your agreement, we may arrange further tests, adjust your medication, or seek specialist advice during our weekly meetings with the Cardiology Team at Macclesfield.

How long can I expect to wait?

The waiting time for an appointment is typically 1-4 weeks.
