Middlewood Partnership Patient Participation Group Constitution

  1. Name

The name of the group will be “The Middlewood Partnership Patient Participation Group (Middlewood PPG).”

  1. Aim of the group

To support and challenge the Middlewood Partnership to achieve its aim of providing ‘the best possible care for our patients with the benefits of modern development and technology’ driven by ‘the traditional values of family medicine’.

In order to support this aim of collaborative development of high-quality practice, the group will.

  • gather information from patients and offer opinion on their views to the Partnership
  • liaise with doctors, practice staff, community health workers, CCGs and others concerned with health care
  • identify gaps in current provision and local needs for service development
  • disseminate information about the Patient Participation Group (PPG) and Partnership
  • contribute to the development of collaborative services.

 

  1. Membership

The Middlewood PPG will be a federation of the four component bases of the Middlewood Partnership: Bollington, Schoolhouse Surgery Disley, McIlvride and Priorslegh; these will be known as `Middlewood Partnership Patient Forums’. Membership of these Forums will be open to any patient, Practice Partners, the Practice Managers, and representatives of all other staff. Every effort will be made to ensure that the membership of the group represents the patient demographic and that a range of patient interests is represented, including adequate representation of other communities in the Middlewood Partnership. Each Forum will organise its own meetings. Over time other Forums may be developed to represent specific patient groups (e.g. patients with diabetes, antenatal groups). The PPG and its Forums will be non- political, non- sectarian and formed using the principles of equality and diversity.

The Middlewood PPG will have a Steering Group consisting of the Middlewood Partnership Managing Director, GP Partner representatives, the Chairs (or representative) of each Forum plus one other representative of each Forum. The Steering Group will meet at least quarterly but more frequently if required.

A note will be kept of each meeting by a secretary recording key issues and decisions for communication to the Patient Forums, PPG members and Partnership patients via the website.

  1. Activities of the Middlewood PPG Steering Group.

As well as being a conduit for information and views between the Partnership and patients, the Steering Group might also: commission activities from the “Forums”, carry out surveys, promote practice and health related activities in the wider community.

The PPG Steering Group will provide a rota of patient members to attend meetings of the Team Bollington, Disley, Poynton Community Services group. These meetings take place once a month and discuss a range of service and strategic issues.
