A guide for care home staff
This page explains how to contact the Middlewood team for any clinical or medication-related concerns about care home residents. Please follow the correct route below to ensure your query reaches the right person quickly.

Clinical Concerns
If a resident becomes unwell, please assess them using your experience and the RESTORE2 tool. If you feel they cannot wait until the routine ward round then please follow one of the three options below.
RESTORE2 Guidance
NEWS 7+ or Red Flags
Suspected fracture, acute bleed, head injury while on anticoagulation, or cannot wait for ward round and no ceilings of care plan (not for admission).
Action: Treat as urgent.
Most likely phone 999 unless you require further advice.
NEWS 4 to 6
Action: Monitor and call for advice if concerned.

Action: Continue to monitor.

Ward Round
Each care home has a named GP and a regular ward round. Your GP and scheduled day will be provided by Middlewood, and any changes will be communicated in advance.
